Forms

All players are required to fill out a signed release form and will be refused the right to play without one. We strongly encourage you to print the necessary forms then sign them before coming to play.

In order to download and print the pdf versions of the forms, you must have Adobe Reader installed.

If you don't have it already, it can be downloaded for free from Adobe's website.

Two options are available to minors:

  • Complete one of the above forms and bring it each time he/she wishes to play
  • For annual participation, fill out the presented form with parent/guardian on-site
